Why didn't my incentive discount apply after a product swap?

Issue
After swapping a product, it shows the wrong price and does not apply the subscribe-and-save discount.
Environment
Platform: Cross-platform
This applies to merchants using Product Subscription Plans.
Resolution
When using Product Subscription Plans, you must select a Plan during the product swap to ensure the incentive discount applies afterward.
